The Best way to make sure your healthcare wishes are honored is to have the Five Wishes document.
The Five Wishes document is a Legal Living Will that can easily guide you through documenting your care and comfort choices. It let's you choose the person you want to make healthcare decisions for you if you are not able to make them for yourself.

Record Keeping Questions
Some people are really organized, while others keep trying every year to be organized.
